I wish to apply for the following CSQ funding Program which is a Construction Skills Queensland initiative.

DECLARATION

I declare that.

I AM An existing worker in the Building and Construction industry, an unemployed eligible worker (been unemployed for less than 4 years) / Career Seeker (SAGT only) or A refugee and humanitarian visa holder

I AM A permanent resident of Queensland and/or I am permanently employed in Queensland.

I AM Working on a Major Project (Major Project funding only)

I AM NOT An employee of any Authority (excluding employees of an Indigenous Council that is a Principal Contractor – Short Courses General Construction only)

I AM NOT Funded by an Authority or such other source for delivery of the same Training being undertaken as part of this Program

I AM NOT Currently enrolled and participating in a school program (excluding eligible school-based apprenticeship and trainees)

I AM NOT A contracted trainer and assessor or an existing worker of any RTO.

I AM NOT seeking a Verification of Competency

I AM NOT an active apprentice or trainee in Queensland undertaking a Recognised Apprenticeship or Traineeship OR an existing worker/unemployed worker in the first year out of a Recognised Apprenticeship or Traineeship OR a cancelled apprentice or trainee who was undertaking a Recognised Apprenticeship or Traineeship that has been cancelled and they are currently assessing State Government funding to complete their institution-based training.

I HAVE NOT Been previously assessed, trained or received funding for the Unit of Competency/Qualification I am claiming for

I HAVE NOT Been previously funded under this program for (three) 3 RII mobile plant operations Short Courses in the same contract period of October 2020 – September 2021.

The following documentation is required for recording your evidence that you meet Construction Skills Queensland eligibility criteria for CSQ funded or partially funded training.

You must be an Existing Worker in the Building and Construction Industry, or Unemployed and provide evidence of one of the sections below.

Please supply identification and residency related documentation to confirm eligibility

You must provide one of the below:

An Australian or New Zealand citizen, a permanent resident of Australia or a refugee and humanitarian visa holder

• Australian/ New Zealand Birth Certificate; (Extract or Full)
• Current passport;
• GREEN Medicare card; (COLOUR COPY REQUIRED)
• Appropriate Visa; (Humanitarian or refugee)

You must have one of the following:

Permanently reside in Queensland or

• Telephone account stating your residential address; or
• Bank Statement stating your residential address; or
• Statement from a Real Estate Agent; or
• Any other formal documentation stating the participant’s full name and current residential address. E.g. Drivers licence;

Be employed in Queensland or work on a Major Project

• Declaration from your employer stating that you are employed in Queensland or working on a MajorProject

If you have previously received funding for a unit of competency or qualification during the contract period of October 2020- September 2021 from CSQ, please have the units listed below.

STUDENT TO COMPLETE

I comprehend and understand that CSQ subsidised funding is available for the competent completion of the Unit of Competency and that I will be liable for the payment of the full course fees if I choose to withdraw from the course or if after completion of the training that I am found to be not eligible. I give my consent for Construction Skills Queensland to contact me for the purpose of a review and conducting a destination survey and/or contacting the for the purposes of advising a Participant of Construction Skills Queensland products and services, whether current or future.
